Track Changes showing on topics not touched

I have an individual who has a document where change bars are showing up on entire topics where we don't have the track changes enabled. Has anybody seen this before?

Details:

We have a DITA bookmap, that map includes sub-maps. Each chapter has its own map. The preface 'chapter' is made up of a lot of boilerplate stuff where we conkeyref in material (things like legal text, and the like). What we're seeing is that automated content is now showing up as changed and in green. I'm beginning to think the original author may have a missing close tag somewhere for the tracking, but it doesn't seem to be in any of the topics that are in the preface section. Would not having a closing tag do this? Any clues on why this is showing up in topics that don't have track changes enabled?

Hard to say, would have to look inside the DITA content to know more about this.
