Cross-reference links not rendering?

The topic I've inherited from a previous writer is a readme with a list of issues, and each list item is a cross-referenced link to the corresponding section. The topic validates, the ditamap transforms correctly, but when I open the index.html output in a browser and click any of the links, I receive the following error message:

Cannot find ...:Users/jive/Desktop/CS11Readme/concept_abg_x

However, if I open the index_frames.html output in a browser, all of the links work as expected.

Is this a known issue with Oxygen XML Author 16? Or is there something obvious that I've overlooked?

Hello,

No, it is not a known issue, if the map validates correctly. Please validate the map with the action called Validate and Check for Completeness which is available on the toolbar of the DITA Maps Manager view. If you get no validation error in the map please send us a set of sample files (a zip archive) for testing them: a sample DITA map and all the included topics.
