Problem with adding a new image in ePub

I am trialling Oxygen Author for epub editing - I'm new to it, so apologies if this questions is an obvious one.

I have added a new image to an epub file, in the image folder (OEBPS/image). I did this by right-clicking on the image and choosing add file, then browsing to the file in my local drive. The new image appears to be present in the zip file - I can see it in the image folder in the Archive Browser and it's present in the image folder if I manually unzip.

However, when I try to validate the ePub, I see conflicting validation errors:
1. image is missing
2. referenced resource is not declared in the OPF manifest
3. item exists in the zip file but is not declared in the OPF manifest

The item definitely is present in the OPF manifest. Path is:
<item id="harbour_low.jpg" href="image/habour_low.jpg" media-type="image/jpg"/>. It also seems strange that the image is flagged as missing, but also as present and not declared?

Very really grateful if anyone can help me ... it's driving me mad!
