Continue to get the error "Topic reference (href : #null) no

I continue to get the error "Topic references (href : #null) not found !" every time I try to publish any kind of topic or map with topics in it. This started occurring after I made some DITA specialization changes. I resolved all errors related to that.
Now, I continue to get this error despite the fact that I actually have no hrefs in my topics, all topic references are correct, and this error does not show up during a DITA validation check.
I decided to try and transform a sample document and every sample that comes with Oxygen out of the box actually produces the same error in the transformation log with the error of [DITA-OT] DITA transformation failed. Try changing the "args.grammar.cache" parameter from the associated DITA scenario - - 1:0. So I created a new PDF transform with this parameter turned off and I continue to get the error for both my current documentation and the samples. I am totally stumped. Is this an OT bug? An Oxygen bug? Thanks.

I also replied you on the DITA Users List.
As specified there I need to look over the entire output generated by the Oxygen console view after the transformation fails.
