Compilation errors in oXygen when attempting a CHM transform

Compilation errors in oXygen when attempting a CHM transform
I have a ditamap with a small number of topics, the map and the topics all validate fine, the map can be produced as PDF with XEP, but when I try to publish it as a CHM, I get:
[exec] HHC5003: Error: Compilation failed while compiling... <topic>
...for each of the topics.
The resultant output of the .chm file shows the toc pane, but when I expand the toc pane from the name of the publication, the topics in the toc pane all have "?" next to them. Clicking on any of the topics results in "This Program cannot display the webpage" in the body pane.
I have searched around a little for help on this issue, but don't see any real steps to take. Does anyone have any experience solving this?

Are you sure you have no broken links in the table of contents?
Please send us if possible the content of the entire Oxygen console:
