Cannot create index in generated PDF

Hello, I'm hoping you might be able to help me troubleshoot a problem I'm having with indexes.

I am authoring in DITA and have used the <indexterm> tag to flag entries that should appear in an index. As an example:
<p>Upload a <indexterm>data set</indexterm> to use when creating a visualization.</p>
I have tried using both PDF transformation scenarios from my manyeyes.ditamap file (using default options for the transforms) and neither will produce an index in my PDF.

I then tried created a DITA book map file and including a specific element in the back matter to bring in the index:
<booktitle>
<mainbooktitle>Many Eyes user guide</mainbooktitle>
<booktitlealt>Using Many Eyes</booktitlealt>
</booktitle>
<chapter format="ditamap" href="manyeyes.ditamap" />
<backmatter>

<booklists><indexlist /></booklists>
</backmatter>
</bookmap>
That didn't work, either.

Do you have any suggestions for how to generate an index in my PDF?
