Duplicate topic in PDF output

Duplicate topic in PDF output
Hi,
When I build my PDF output, it repeats one topic 4 times (i.e. the one time I have it in the map + 4 "phantom" entries.) The phantom entries appear in the TOC - but without page numbers and only the real reference is generated in the output.
Could this be due to using a bookmap within a DITA map (I'm trying to transition to bookmaps so there is some inconsistency so far.)

We publish lower-level <bookmap> maps as a part of higher-level <map> maps in a couple of configurations, and they publish correctly for us.
Which PDF transformation are you using?
I suggest making a copy of your content, then steadily deleting content to narrow down the problem. When you change or delete something that makes the problem disappear, put that part back and delete something else instead. This will help you narrow down the root cause of the problem. And if you can't figure it out, at least you have a nicely reduced testcase for someone else to look at!

As Chris mentioned, could you indicate the transformation scenario you are starting from and the Oxygen version you are using?
Also, as in Chris recommendation, could you try to reduce the file sample where the problem occurs? With this narrow sample, could you identify the issue?
If you still cannot find the root cause of the problem, you can still send us the reduced sample at support@oxygenxml.com

It's the DITA Map PDF - based on HTML 5 & CSS transformation.
When I publish it alone, I get 2 blank pages at the top of the output. When built as part of the main documentation, the initial topic is flipped with it's first child and that is the topic that is repeated 4 times.

Unfortunately, it's impossible to analyze this problem from the forum, you should send us a sample at support@oxygenxml.com.
