Unable to insert keydef from DITA Maps Manager

There is no option to insert keydef from the DITA Maps manager or Editor window of the Oxygen XML Author 21. Switching to text mode and adding a XML element is only option. From the documentation, I understand that we can insert keydef from "Insert" contextual menu option. However it is not working as expected.

Are you editing a DITA Bookmap or DITA Map? Do you have your custom DITA specialization for the DITA map you are editing? Maybe your specialization disallows the use of the key definition.
In the DITA Maps Manager if you open a DITA Map and right click on a reference to a topic you have those sub-menus "Append", "Insert before", "Insert after". Inside those sub-menus you should find a "Key definition..." entry. The entry is missing only if you cannot insert a key definition at that specific location. For example in a bookmap you cannot insert a key definition on the first level of the map.
