[oXygen-sdk] Not able to use insert symbols

Hi,
  Whenever I try to insert a symbol using the symbol toolbar, I get the
following error which I am not able to debug:

System ID:
/Users/xxxxxxx/.../dev_guides/ajax/sforce_api_ajax_debug_shell.xml
Severity: error
Description: :Paste failed due to: Index: 0, Size: 0

Could someone point me in the right direction on what is happening here? I
did create a new class by extending the AuthorExtensionStateListener and
implemented the inserText and inserFragment methods which work fine.
