No, M20 of the 1.5 Toolkit doesn't do it either. I've already logged a bug
for that and provided a test data set.

I also noticed that the Toolkit isn't producing links for <keyword> elements
that use @keyref. I've sent mail to Robert Anderson asking if that is on
their implementation plan or if it's a bug.
