error with comment form

With Oxygen's webhelp with feedback, somehow the comment form got messed up. It doesn't appear in the display, and I received this error:

Any idea how to fix this?

Hi Tom,
We need some more details because we don't know how to reproduce this error. What values did you set for the parameters
webhelp.product.id and
webhelp.product.version when you executed the Webhelp-Feedback transformation in the Oxygen application ? Did you use a " character or a ' character in any parameter of the transformation (in the Oxygen application) or in a text box displayed in the installation (in the browser, for the server deployment)? Did the installation in the browser report any error or warning?
What action did you do last time in the Webhelp page when you get this error: press the
Submit button for adding a new comment, press the
Log in button in the login dialog box, etc?
Please can you
send us the following file from your Apache server installation:
[Apache-dir]/mysql/data/mysql_error.log?

Sorry, I'm not sure what triggered the error. I didn't use any special characters. I'll be on the lookout the next time it happens to see if I can pinpoint the cause. I ended up repurposing the db for something else, so the error log won't be any help at this point.

We will try to pinpoint the error cause if we have more details from you. If the comment form in the Webhelp pages gets messed up again please let us know and if possible send us the error log file and we will try to find the cause and fix it.
