Create fillable forms in PDF?

We're currently authoring in DITA and transforming to XHTML and PDF. Previously, we had these worksheets for customers to fill out that we used to publish as Word so customers could complete the forms offline. I'd like to skip Word, and send them PDFs with fillable forms, but I can't identify a way to do this.

Anybody know if this is possible, and can provide a link to examples if so? I can't track this down myself, so far.

Hi Mike,

We do not have experience with PDF forms. From what I think commercial PDF processors like Antenna House and RenderX XEP have such support.
Maybe you should register, search (or ask around) on the Yahoo Groups DITA Users List, for example this previous discussion:

Thanks, Radu. I'll follow up with that information but it does not appear that what we want is possible/easy enough.
