Michael, you have other people attending to your question and I'm sure you'll get it worked out.
I'm writing, delightedly, to highlight the awesome job you've done of posting a small, simple representative sample of input and transform. *I* should learn from this! We recently had a case of a poster's "simplified" re-post being so voluminous that its initial respondent's incredulous, dry comment "This is the 'simplified' version?" was hilarious. *Your* re-post, by contrast is a study in economy!

OK, here you go. I'll give a simple input xml, along with the main bits of the stylesheet. Output is intended to be abstract types definitions for a wsdl.
Let me know if you would prefer file attachments.
