I am sorry I am bit new to xslt but isn't the / template joining all the
Item elements?

 <xsl:template match="/">
         <A>
            <target>
                 <xsl:apply-templates select="//Item"/>
                 <xsl:apply-templates select="document('File2.xml')/
*//Item"/>
             </target>
         </A>
     </xsl:template>
 </xsl:stylesheet>
