So the next question is: will the PDF be fully hyperlinked with
links from the TOC to chapters/sections/tables/figures, hyperlinked
to external web sites, and hyperlinked from the index to items on
the page?

A suggestion in addition to all the above that has worked well for my PDF book customers is to hyperlink the specification section references to the web copy of the specification itself. Particularly handy in the XSL-FO book, when there are nuances needed that aren't covered in the material I've written, users can click on the section reference and up it pops (once cached) in a web browser, right at that section number where more details can be found.


The feedback we get is that this adds a lot to training material, so adding it to Mike's content may help a lot.
