The XSLT 2.0 construct

<xsl:for-each-group group-starting-with="h2">

makes this kind of exercise much easier. IIRC there's an example of this
usage in the XSLT 2.0 specification. 

In practice, the difficulty depends very much on how regular your input is,
for example whether the existing h1/h2/h3/h4 headings are properly nested.

If you need to do it in XSLT 1.0, then it's quite a bit more difficult.
Search for articles on "positional grouping" or "sibling recursion".
