IIRC, we (the XSL WG) looked into this during the development of 2.0 and
decided that a strict reading of 1.0 did not say that the "null namespace"
could be mapped (because it isn't actually a namespace), despite the fact
that most implementations had interpreted the spec as allowing it. But
basically, the 1.0 spec is silent on the point.
