  If you use a predicate in the select of the xsl:for-each, instead of
an xsl:if, you'll get the same position in the node set and in the
output tree:

    <xsl:for-each select="/NounOutcome/DocumentIds
                            / DocumentIdType[Id = '43]">
      ...
    </xsl:for-each>
