You have two definitions of the default namespace, both pointing to different uri's. The one on the xsl:stylesheet element is not being used as far as I can see. What happens when you remove it?
Another try could be not to use default namespaces, but giving the namespaces a prefix and adding an 'exclude-result-prefixes' attribute to the xsl:stylesheet element.
