In this case, you have a number of options:

- use <xsl:key ...>
- use generate-id()
- use an XPath expression like:
  <xsl:apply-templates select="Item/Field/Value[not(. = preceding::Value)]"
/>

On big XMLs with a lot of nesting, the first definitely deserves preference.
