Because (I think) my <insert> needs to be replaced by a series of <name>s (*not* replaced by the <extra>), and it's those <name>s that need the numbering, not <insert>.
Or am I missing something?

This is cool, but could you explain again for the curious why
<xsl:number count="name|insert"/>
wouldn't work just as well?
