I've been using Cocoon for a while and I'm _very_ pleased with
it. I can concentrate on writing doc and not have to worry about
the serving.

Yes, it caches the generated HTML so that frequently-requested
URL don't have to be reconverted every time. I don't know how
big the default cache is, though, nor what the persistence is
(my use of it is not at production levels).
