In mixed content such natural interpretations are very dangerous and 
shouldn't be done. It is quite common to use inlined elements to carry 
out additional metainformation, for example index terms in DocBook can 
occur almost everywhere:

<title>Foo Baring<indexterm><primary>foo</primary></indexterm></title>

With this markup approach title can't be simply accessed using it's 
string value (in XPath sense). It is hassle for processing, but there 
are document types where you can't live without annotations, indexterms, 
footnotes, ... that can occur almost everywhere.
